Three stories, clapboard, front door with pediment above, gable roof, two story extension in the rear. Carriage house with cupola on the left. Fence with gate in front, small trees in front yard and larger trees on the right. View of front and left sides. Title printed on original below: "Birthplace of Ann Hasseltine, Bradford, Mass. / Wife of Rev. Adoniram Judson Missionary to Burmah." Inscription on mount below: "Hazeltine House."
